Eligibility Criteria

Additionally, a patient must fulfil the following eligibility criteria:

• Be unable to adequately perform self care and/or healthcare needs.


• Be temporarily or permanently homebound.


• Require supervision assistance while recovering or recuperating.

Individuals who have private insurance and workers compensation insurance are eligible for admission upon authorization of the insurance provider.

• Private Pay/Insurance


• Private insurance
